INFORMATION NEEDED:  IF YOU DO NOT RECEIVE FOOD STAMPS
Identification
Birth Certificates for all children
Social Security Cards for everyone in household
Proof of all income (wages, welfare, food stamps, etc.)
Proof of all expenses (rent receipt, utility bill, phone bill, etc.)
Proof of current address; all information must be current
CLIENTS THAT RECEIVE FOOD STAMPS ARE INSTANTLY APPROVED
 (NEED CURRENT FOOD STAMP PRINT OUT)
PARENTS CAN APPLY FOR CHILDREN AGES NEWBORN TO 12 YRS
